Complete vs. split?: Peroneus longus tendon injuries are more common in 40+ yr olds. The tears can be complete or longitudinal splits down near your ankle. If rest, therapy and nonoperative treatment fail to improve symptoms of pain, surgery may be indicated to debride the injured tissue or repair the tear/avulsion or longitudinal split. Extent of tear?: The vast majority of partial tears are initially treated with immobilization using a splint for 2-4 weeks with possible therapy. If the tear is complete or persistently symptomatic after non-surgical treatment, surgery may be indicated.

Surgical repair: Peroneal tendon tears rarely heal, scar tissue, adhesions and synovitis of the tendon usually results. If pain is present for a protracted time one should consider an mri. If the tendon is determined to be partially torn, surgery should be considered.
