Medicare: Usually, when someone is hospitalized for acute illness or surgery, and needs rehabilitation after discharge for reasons such as deconditioning, weakness, inability to restore independence with activities of daily living, or needs nursing supervision of treatment. Call SNF: You can call the nurse manager/case manager/nurse liasion in the nursing home of your choice, they will evaluate the patient's eligibility on clinical gorunds using insurance criteria.
