See the doctor: An anal fissure can cause local pain and bleeding from the anal region. See your doctor for medications to try. Avoid constipation, avoid straining at the toilet, use stool softeners from the drug store. Surgery is sometimes indicated for chronic fissures that do not heal.

Fissures: Can cause pain and bleeding. Depending on whether he has received any legitimate medical treatment and corrected his diet and fluid, then he may be a candidate for surgery ti fix it. See a colorectal surgeon for proper treatment as there are several options available before considering surgery. But if all else fails surgery will resolve it.
