Could intermittent flank pain be a sign of renal failure or something else?

Not renal failure: Intermittent flank pain may be related to kidney stones. Could also be related to a kidney cyst or chronic pyelonephritis but not kidney failure. Only way could be sign of failure would be if you had lost virtually all your kidney function in both kidneys and had an obstructing stone in one kidney.

Flank pain: More likely a sign of kidney stone. May be completely unrelated to the kidneys and just be muscle spasms.

Flank pain: It could. It could also mean many other things - kidney stone, kidney obstruction, infection, etc.. See your doctor and have this investigated.
