Culture and antibiot: The "bug" should be identified in joint fluid and it's sensitivity to various antibiotics determined. Usually IV antibiotics are initially used, but a septic joint can lead quickly to arthritis, and in some cases orthopedic intervention is required. Paradoxically, I do see overdiagnosis of strep septic joints because it is a common contaminant from skin.
