Which type?: Is this due to testicular failure or to hypothalamic/pituitary problems? The main issue is whether or not the testes could possibly work and if fertility is desired. So need to know the cause of hypogonadism.

Fertility or not?: Hypogonadism (low testosterone levels) can be because the testicles are not functioning normally, with high fsh and lh levels, or could be due to low fsh and lh levels (hypogonadotropic hypogonadism or hypo hypo). If trying to conceive with hypo hypo we give HCG and fsh shots. For other causes of hypogonadism we can give testosterone replacement but not if you're trying to conceive ...

Opioids and low T: In response, Methadone can suppress the pituitary's gonadotropins which would then decrease testicular production of testosterone. Decreased libido, some breast development, and mood changes otherwise can happen. Treatment may include supplementing with testosterone. But, decreasing Methadone or using Buprenorphine instead may also help per what i understand. See endocrine and addiction specialist.
