Hematuria: Painful causes: stones, infection painless causes: renal cell carcinoma (very unlikely at 18), arteriovenous malformation of the kidney or urinary tract. You need to see a urologist and likely have a benign cause.

Need imaging: Antibiotics for gross painless hematuria is inappropriate.I am surprised no imaging (That is Xrays) was recommended. Do not ignore it dear.
