Not the exterminator: If something has already died, an exterminator wouldn't be needed. Anything with that bad of an odor needs to be checked out, to see if it is an infection with anaerobic bacteria or an area of dead, gangrenous flesh. Such a problem can lead to fatal complications if left untreated.

See doc: If hygiene is not an issue, you should see your physician to get this checked out to rule out infection, pus pocket..Your doc might send you to a specialist as necessary (but not an exterminator :-).
