May be adhesions: You may have adhesion s due to scar tissue, diagnosis of incisional hernia is not difficult in most of the times except in obese people.A small hernia is difficult to feel the defect. An ct scan will show abdominal wall defects.

Sometimes: An incisional hernia is a defect in the abdominal wall at the site of a previous surgical incision. They are usually straightforward to diagnose by physical exam in your surgeon's office, however in certain cases they are difficult to diagnose (obesity). A diagnostic laparoscopy is helpful in these cases. If a defect is visible, it is repaired. Occasionally, however no true defect is found.
