Steroid shot: See a pain management doctor and get a steroid shot to the space and see if pain improves. If not, need to see a neurosurgeon to talk about discectomy.

Several things: First realize this is a common level for a disc to herniate & that most get better over time with treatment that can include: traction, activity modification, medication & sometimes epidural steroid injections. If symptoms persist after 12 weeks of treatment & are severe, surgery can be indicated with a 90% success rate done by a orthopedic spine surgeon or neurosurgeon if a nonsmoker.
