Elevate : The legs and apply hot warm compresses to the region of gout. If antiinflammatories are not contraindicated i would prescribe. Due to pvd, i would likely not inject the region.

Meds, etc.: Typically with PVD and gout you will be put on medications to decrease the uric acid and gout. Though probably more effective would be a cortisone injection to the area to reduce the swelling and redness and symptoms. See your podiatrist.
