Can you get a breast reduction procedure which removes only fat and not milk ducts?

Yes: Liposuction can remove only fat from a breast and nothing else. However, not everyone is a candidate for liposuction reduction. While breasts feel fatty, they may be mostly glandular tissue which doesn't suction; this is more true in younger women. Also ls doesn't tighten tissue so fold irritation can continue. Many woman can breast feed after regular reduction if the nipples are left attached.

Yes: There are a number of different techniques for breast reconstruction, many of which remove fat and breast tissue but leave milk ducts intact. While i caution my patients that i cannot guarantee that they will be able to nurse postoperatively, many patients are able to.

Good ?, but no.: Breast tissue includes fat and glandular (ductal) tissues mixed together, though there is a layer of mostly fat just below the skin surface. If liposuction was confined just to this layer, a small volume of breast volume could be removed, though with a higher chance of ripples or ridges that could be seen or felt. If you're thin even this won't work. See an expert ps for advice. Best wishes!

Probably not: The person with truly excessive breast size will have an over abundance of glandular tissue which doesn't respond well to anything other than removing excessive glandular/fatty tissue. Liposuction reduction usually leaves the large amounts of skin behind as well asptosy.
