He's alcoholic: Alcoholics have limited ability to take of their health or needs in a healty way. And they have many irrational reasons why not or they just don't care. He could be depressed and has suicidal thoughs, or he does not see the significance of his problems, he is in denial of them. This does not just happen in alcoholics by the way, but is usually more pronounced with them. Seek professional help.

Passively yes: While he is on a road to death quicker than others that doesn't mean he is actively suicidal. As a brother you can model good health and have open conversations about your concerns. Ultimately the decision to change will be his and you will have to accept that along with his potential untimely death. Keeping your feedback from being judgemental will give you the best shot at being heard.

Alcohol/ HTN: What worries me is not taking his medication for his blood pressure, which can lead to stroke or cardiovascular complications. He needs to have thorough assessment by his physician who then can show him facts & talk about compliance issues.
