Sports med doc: I would recommend a sports medicine physician. The option includes a non surgically trained sports medicine physician who is skilled at evaluating musculoskeletal injuries with a focus on conservative treatment options. If an injury is deemed to require surgery an appropriate referral is made. The other option is an orthopedic sports medicine physician who specialize in surgery.

Several types: Sports medicine physiatrists specialize in sports injuries as do sports medicine orthopaedic surgeons, and primary care sports medicine specialists. Many hospitals and clinics have a sports medicine department. Sometimes you can start with your pcp and get a referral.
