Laparoscopically: Using slender instruments and cameras through small ports or tubes, the stomach is stapled and divided. In the lap sleeve the outer 85% is removed and the stomach is reshaped into a thin banana shape. In lap gastric bypass the stomach is divided leaving a small pouch of stomach that is connected to a limb of small bowel and the food diverted. The rest of the stomach remains but doesn't "see" food.

Small holes : Both bypass and sleeve are laparoscopic (small holes). Both use special staplers which are standard. For the bypass, a pouch is made by dividing the upper stomach. Then the small bowel is divided and the far part is brought up to the pouch. The sleeve uses staplers to make a thinner stomach. Both take about one hour.
