Yes: Protect your hearing. Reduce noise levels if possible - watch the level you use a music player (like an ipod). Wear ear plugs or hearing protection when around prolonged loud noises - mowing the lawn, riding a motorcycle, etc.

Depends on the cause: Correctable causes are mostly conductive hearing loss . Ex. Ear wax, otitis media, eustachian catarrh, otosclerosis, perforated ear drum etc.
