Hydrocele: The answer to this question would be based upon other symptoms. Are you in pain? Is the scrotum or hydrocele growing? Do you have any difficulties or changes in your urine? Is the pain treatable with an anti-inflammatory medication? Most hydroceles are benign and the aching that they can cause is only temporary. But, sometimes they are so painful or complicated and need to be operated on.

Scrotal ultrasound: Most hydroceles are benign & only need treatment if symptomatic. However occasionally a hydrocele develops in reaction to a testicular tumor. Would be a shame to miss a testicular tumor which has an almost 100% cure rate if diagnosed & treated early. Hence advice regarding need for an ultrasound unless hydrocele is soft and testicle feels normal, ie similar to your left testicle.
