Step by step.: First congratulations on breaking the habit. Start by getting a physical with your doctor so that you can be cleared to get back in shape. It is going to take time and you need to be patient. A personal trainer can help guide you to do it properly and stick with it. Good luck.

Start slow.: Don't hit it too hard too fast. Start slow, maybe twenty minutes of light cardiovascular work to get your feet wet. As this becomes easier, lengthen your sessions and go harder. Start with twenty minute sessions and increase duration and intensity from there. Quitting smoking was a great first step.
