Obstruction of flow: A mass in the third ventricle could either obstruct the ventricle or severely limit the flow of cerebral spinal fluid (CSF) from the lateral ventricles to the 4th ventricle. This reduction in flow or obstruction of flow causes the CSF to well-up behind the obstruction producing increasing pressure and hydrocephalus. Relieve the obstruction or shunting the ventricle will relieve the hydrocephalus.

Colloid cyst: The mass blocks the normal circulation of cerebrospinal fluid, and like a dam, there is a backup of fluid with pressure within the ventricles which expand like a balloon. Excise it or use shunt to release pressure and symptoms reverse.
