Depends...: Depends on your pain sensitivity or threshold for pain. Most patients would say the momentary pain during the actual needle piercing skin is 6-7 out of 10 (with 10 being the most painful). The pain does quickly subside however.

Variable: Each person has a different tolerance level for pain. The pain caused by a piercing depends on body location. Ear lobe piercings are usually tolerable. Piercings through cartilage hurt more. The tongue can be quite painful. Nipple & genital piercings have been described as very painful. I am sure that there are exceptions to the above generalizations.
