Depends how loose: Depending how loose the tooth is. Sometime with a gentle pull using dental floss rapped around the tooth. Usually eating an apple can finish loosening a tooth. However the best advise is visit your family dentist.

Keep at it: Keep rocking back and forth until it breaks free. If it is really loose then loop a piece of floss around it and pull both ends apart and the tooth should pop right out. Just be sure that the child's head is facing down in case the tooth wants to shoot toward the back of the throat. For a real creative way see this youtube video. http://www.youtube.com/watch?v=bphrianw4uq.
