Maybe: We really need more information. It would be unusual for a four year old to be normally losing a tooth. More likely some pathology is involved and then yes the tooth should be extracted. That said sometimes we allow loose teeth that are going to be lost prematurely to fall out on their own. This situation most happens when a tooth has previously been restored with a pulpotomy and crown.

Why is tooth loose?: If loose because of decay/abscess may need root canal rx ; crown or may need extraction and space maintainer. If permanent teeth erupting prematurely space may be an issue. Please see your dentist or pediatric dental specialist asap...Abscesses are nothing to mess with. Also ask if orthodontic consultation is in order. It's never too early to find out what's up.
