Can having a low hiv viral load lower the likelihood of transmission by a lot?

Yes: And this is helpful in limiting the spread in the community, but nobody should rely upon that and should always practice safe sex.

Yes!: Yes, by a lot. A large clinical trial showed that treating an infected person reduces the risk of transmission to a negative partner by 96%. If the positive partner has an undetectable viral load, the benefit is probably close to 100%. We still recommend condoms though, since you don't get your viral load checked every day.
