When she is ready: Hospice is an individual choice; people with serious illness that is life threatening, with less than 6 months left to live are eligible. If she does not want treatment and/or is ready to be made comfortable, she is ready to talk about hospice.

Now, probably: How old is your grandmother, how functional is she, is she going to get treatment for the cancer or not? If she is debilitated and desires comfort care only, the answer is now. If she is with it, ask her what she wants and proceed accordingly.
