No: Any break in the skin can leave a scar, but small puncture wounds will not be noticed in most people. If a person is a keloid former, then even something as small as a mosquito bite can cause a visible keloid scar.

Immunization scars: Immunizations given in the us generally do not leave scars unless there is an allergic reaction or infection. Some vaccines given overseas or that used to be given previously, like the bcg vaccine for tuberculosis (also used for other infections or as immunotherapy for bladder cancer) or the smallpox vaccine do leave scars. Injected polio vaccine could also leave scars, now the oral form is used.
