Obstruction: If there is labored breathing after the trach, then either there is lung disease, or the trach tube is partially blocked. Your surgeon should be able to figure this out and help.

Possible causes: I agree that there may be an obstruction, and granulation or scar tissue may form over time. Acute sudden changes are often related to mucus plugs or an infection.
