Several poss. causes: Common causes of flank pain such as organ problems or disc injuries can be found by your average doc. A good osteopathic exam, however, in addition to the standard tests, might define more obscure causes. Ex: in my practice i've seen flank/back pain of 'unknown cause' lasting 10-20 years when what they have is a quadratus lumborum muscle spasm which is easily treated, if you know to look for it.
