Poor blood supply: The simplest answer is things that hurt blood supply to this area - usually from drug use (especially vasoconstrictors like Cocaine or meth which can be snorted). Other causes can include infections or diseases like wegner's ganulomatosis - often treating the underlying cause can help and if the situation stabilizes this can be treated by a surgeon.

A few things: Cocaine, previous nose surgery, other types of nose trauma, chronic steroid sprays, and certain diseases such as wegener's granulomatosis can cause septal perforations.
