Partial or total hip: Replacement if it is displaced. If it is just a crack and not shifted out of position surgery can be done with three screws to keep the femoral head from shifting. Usually it is displaced and needs a replacement because once it shifts the blood supply to the head is irreversibly damaged.

Surgery: In a young person with a femoral neck fracture the fracture should be fixed with screws in an anatomical position. This can be done in some circumstances through small incisions but a large formal incision may be required to get the fracture aligned in perfect position.
