What can cause scattered pulmonary parenchymal opacities in the upper lung?

Many things: A number of different things can cause this, from tumors (often metastatic), certain infections to autoimmune diseases. You will need additional studies such as a ct scan of the chest, blood work, and likely a visit with a pulmonary specialist.

Everything: Well almost everything. Certain infections, inflammatory diseases, pulmonary edema, scarring, and some "allergic" reactions to environmental exposures. distinguishing among these is impossible without more information. If you had an x-ray, talke to the doctor who took it, so they can review your history, and other findings on physical exam etc. to help narrow down the cause of this finding
