Therapy: Go to a physical therapist to give you exercises to correct posture. Braces and even taping can sometimes be helpful to help re-train the spine.

Bad habits: Bad posture is simply a bad habit. You need to modify your "behavior" and consciously correct posture. Imagine balancing a book on your head when you walk/sit/stand. This is the best awareness exercise i know. Also, do you really have bad posture? Or is it scheuermann's kyphosis which is a structural deformity of your spine which can be mistaken for posture? Xrays or exam would help to determine.
