Usually before.: Sentinel lymph node biopsy is usually done at the time of lumpectomy for breast cancer. The information gained is used in the decision making as to whether adjuvant chemotherapy should be given to increase the likelihood of cure. Rarely, it might be done after chemotherapy if the initial role of chemo was to shrink the tumor to allow surgery (neoadjuvant chemotherapy).
