A lot: First, it's not limited to just psychiatric hospitals. If the doctor is able to get commitment orders from the judge, then the patient is required by law to remain in the hospital until deemed safe to themself and society to be released. If not committed, then the patient can leave against medical advice whenever they want (as with physical medical conditions).

Depends on..: Your state's mental health laws. Most states can keep you in hospital if you are clearly a danger to yourself or others while they obtain a commitment order. Medications can be given to you against your wishes in an emergency or after a judge has deemed this appropriate and provided the hospital with a court order.
