Depends: You will just have a weak shoulder, and the repair might not be as successful if you wait too long. Many older people elect to just live with it.

Rotator cuff tear: When a rotator cuff tears, it cannot heal spontaneously. If left not repaired, the tear can get larger, and can retract further away from bone. Ths makes eventual repair less predictable in terms of ability to repair, as well as healing of a repaired tendon.

Pretty severe: I would first question whether a 24 year old has a torn rotator cuff. Would be very uncommon and only traumatic at best. I would want to know if full thickness or partial thickness. Full thickness tears will progress and get larger. There will be a time when the repair can't be done. This equates to a poor outcome. Partial tears you have wiggle room and don't necessarily need surgery.
