What information is requested normally in a health insurance application?

Basic information: It may be important to know that dental insurance can have pre-existing clauses, such as the length of time since a crown, bridge, extraction or filling was done. Information about date of last periodontal scaling & root planing, (deep cleanings) may may be requested. Social security numbers may be requested for identification.

Dental insurance: Health insurance usually has no dental benefits. Dental insurance will require a full mouth of radiographs for getting an approval ( pre-determination ).
