Kidney ischemia: Coarctation of the aorta leads to decreased blood flow to the kidneys. The kidneys then release chemical mediators which increase blood pressure to try to improve blood flow to the kidney. The blood pressure will stay elevated while the coarctation is untreated.

Kidney's: Coarctation leads to decreased blood flow in the lower body. The kidney's sense the blood flow and release hormones to regulate blood pressure. If kidney blood flow is low, they release hormones to raise BP and vice versa. And after time, the blood vessels in the body respond to the high blood pressure with abnormal changes of their own.
