See Peds: I don't think this is serious, there may be an ear infection, sinus infection( rare in. 4 year olds), but could also include a whole host of head and neck conditions that need more diagnostics. Take your child to a good pediatrician or ENT doc for a thorough examination and next steps to find out the cause
