Diarrhea: That is not preceded by vomiting and that is associated with fever is more concerning. If the diarrhea has mucous or blood in it, then it certainly needs to be evaluated by a physician and consider stool analysis for bacteria and possibly parasites (if there is a recent travel, particularly to outside the US). Make sure he is drinking plenty of an electrolyte solution such as pedialyte
