Ear infection: Ruotured ear drums do occur when extreme pressure builds up from infection in the middle ear. The hole usually heals well. If pain persists it suggests that even though the pressure has be released something else is going on. Spread of infection to the mastoid bone behind the ear is a real possibility and it is definitely time to go to the doctor.
