Many things: Is your baby a newborn? Or is he a 4 month old? In the former case there are several issues, breast anatomy, technique, infant temperament, potential infection (in a lethargic infant) can play a role. In an older baby, teething, colds, other illness can play a role. Try a lactation consultant to help you sort out these issues. Ideally you want the tip of the nipple in the back of baby's mouth.
