Not Usually: Feelings of unreality tend to be episodic and do not always accompany feelings of anxiety. However, some people with panic attacks will report feelings of unreality with just about every episode.

Not usually: Derealization is a symptom of high anxiety characterized by a feeling that one's surrounding is not right, unfamiliar or unreal. It can be very upsetting but it usually does not last too long but can vary. Can be other things. Need a good differential diagnosis.
