Whether or not my back pain is nociceptive, why does neurontin (gabapentin) not work when having a painful episode? only tramadol helps.

Medication: All medications have limitations. Neurontin (gabapentin) is a calcium channel modulator and this is the reason when the pain is really bad the TRamadol helps. TRamadol is an analgesic.
