False negative: A false negative test is occurs when the test result is negative but the patient is pregnant. A false negative urine pregnancy test can occur if: the urine is too dilute, the test is not performed correctly, the test was done too early or there are interfering substances. A blood test may be negative if the hCG hormone level is very low or if interfering substances are present. No test is 100%.

Faulty testing: Either the blood and urine testing product or process was defective or the ultrasound may or may not be accurate. If a defined gestational sac could be seen on ultrasound and heart tones heard, then it is the former not the latter.
