Many: there are more false positives than true positives and that is why CA-125 should NOT be used as a screening test. other causes-- any infection or inflammation , any benign tumor, endometriosis, other malignancies.
Dr. Jason Boardman
General Surgery 25 years experience
Many causes outside of ovarian cancer. These include gastric cancer, primary peritoneal cancer and of course false negative results.

Does an elevated ca 125 level always indicate that ovarian cancer is present?

NO: As a screening marker for ovarian cancer this test is of no use. It can be elevated in women with conditions such as endometriosis, pelvic inflammatory disease, and uterine fibroids to name a few. Its true utility is in monitoring for cancer recurrence.

Ovarian cancer ?'s: Elevation in Ca 125 can be caused by ovarian cancer outside the ovary ( i.e., intraperitoneal metastasis). There are other causes of elevated Ca 125, but see your physician or a gyn oncologist to be certain as soon as possible.
