When you've finished a months worth of amitriptiline are you supposed to have a week break before you start the next packet?

Usually no: Normally there is no one week break after using amitriptyline for one month. Some people may use amitriptyline for a short time, some people may use it every day for a year or longer, but people do not normally take a week long break after one month and then start again. An exception to this could be if the symptoms it was meant to treat disappeared.
