FDA - 200mg/day: However, everyone is different in how they tolerate this and many other similar drugs. As a result we titrate them up slowly usually no more rapidly than 25mg per day every few days. Typical migraine prophylaxis dosing is 50 - 100mg twice a day. Some people tolerate less and it can be used off-label at higher doses if it is helpful and side-effects are tolerable/controlled.

Topamax (topiramate): For adults & children over 12 years of age, it would be 50mg. twice daily.
