Reaction 2 particles: from the cotton that is often used to filter heroin (and other drugs) once they are dissolved in water, and usually heated over a flame ('cooked'). The cotton particles cause a foreign body/immune response, especially in the lungs, with cough, fever, chills, sometimes chest pain and shortness of breath, as well as other autoimmune symptoms - muscle aches, weakness, malaise (french for feeling bad)
