Almost never: Almost all young adults have at least two. If they're not fixed to the surrounding structures, grotesquely big, or rocky / rubbery, leave them alone and forget about them.

Not always: Depends on other symptoms, age, family history, etc. You're pretty young, and cancer is rarer than in older age groups. However, see your doc and be sure that there is nothing wrong and if they don't go away after 6 weeks or so get checked again. You can always get a second opinion if you have not been reassured enough by your doctor.
