Usually clinically..: ...by history and examination. Delusions are false, fixed beliefs not shared by ones culture - the CIA is spying on him/her (non-bizarre delusions) or being abducted by aliens (bizarre delusions). There are also erotomanic delusions (someone famous is in love with the patient), delusions of grandeur or delusions of persecution. Psychological testing can help in diagnosis in unclear cases.
