Check Mg++ / K+: If either is low you can get muscle cramps. Additional tests would include kidney function, thyroid function. assessment of circulation in the legs, EMG-NCS and possibly an MRI of the lumbar region. Rule out the obvious first ( on HCTZ (hydrochlorothiazide) )

Likely cramps: See your internist and address your back pain, and obesity. An exercise program may be beneficial, and OTC supplements containing magnesium, calcium, and zinc might also help. Nothing mentioned so far would have me concerned regarding MS or Parkinson's disease.
